Student applicants-please contact your school Monitor so they can complete the google form for approval.
- The group is voluntary, student initiated, and student led.
- The group has a minimum 10 participants.
- Parental permission form has been obtained for all student participants.
- The group has a constitution, by-laws, or operating procedures.
- The group's purpose, goals, or activities do not promote, encourage, or condone, directly or indirectly, participation in any conduct by students that is contrary to state or federal law, or that poses a risk to their health, safety, or welfare.
